Affinity Community Care partners with individuals with disabilities and complex needs to help them achieve optimal quality health, affordable housing, and meaningful community life. We work with adults who live in the community and want to remain independent, and others who live in a Maryland nursing facility or are homeless and wish to move to a home. We also help people with specialized healthcare needs and disabilities obtain high-quality, medically appropriate healthcare services in the community. Our highly skilled coordinators navigate complex systems and connect individuals with the necessary support and services to thrive at home. Â Using a person-centered planning approach we work with each individual to customize a plan of care that is centered on achieving personal goals and aspirations. Â Here are ways we can help:
Ways we can help:
- Locate and secure affordable and accessible housing, provide housing supports and train individuals in the area of good tenancy topics
- Assist with the transition from nursing facility to a home in the community
- Help coordinate medical appointments and assist with obtaining durable medical equipment and medical supplies
- Develop a creative person-centered plan based on needs, preferences and goals to help ensure health and safety and facilitate community inclusion
- Help coordinate personal attendant care providers to assist with activities of daily living (ADLs) or instrumental activities of daily living (IADLs)
- Assist with obtaining environmental assessments, home delivered meals, and assistive technology
- Help explain services, supports, and options available through Medical Assistance (Maryland Medicaid) and locate other non-Medicaid/community resources
