Home & Community based services
Community Services
Affinity Community Care provides support planning services to older adults and individuals with disabilities who live and want to remain in the community or are living in a Maryland nursing facility and want to move to a home. Home and Community-Based Services (HCBS) are person-centered care delivered in the home and community. Home and Community-Based Services programs address the needs of people with functional limitations who need assistance with everyday activities, like getting dressed or bathing. Home and Community-Based Services are often designed to enable people to stay in their homes rather than move to a facility for care.
Programs and services include:
- Home and Community-Based Options Waiver

- Community First Choice Program (CFC)
- Increase Community Services (ICS)
